  • 1 pet = 5% discount on base medical or wellness plan

Pet Insurance From Nationwide – Plan Options
We think all pets deserve to live life to the fullest. That’s why we’re there for the big stuff, the small stuff, and all the stuff in between. Nationwide is now offering an expanded suite of affordable and fresh, new innovative products. Here they are:
Major Medical Plan (Comprehensive) (starting at $13/paycheck, or $29/month)

  • A low $250 deductible.
  • Covers accidents including poisonings; cuts and broken bones; common illnesses such as ear infections, rashes, vomiting and diarrhea; serious/chronic illnesses including cancer, diabetes and allergies; and hereditary conditions.
  • Also covers procedures and services including surgeries, Rx medications, testing and hospitalization.

Pet Wellness Plan Plus (Everyday care) (starting at $10/paycheck, or $21/month)

  • No deductible
  • Maximum annual benefit $500
  • Two physical exams per policy term
  • Behavioral exam and/or treatment
  • Vaccination or titer
  • Heartworm or FeLV/FIV test
  • Fecal test, deworming
  • Nail trim
  • Microchip
  • Flea control or heartworm prevention
  • One additional test per policy term – choose from health screen (blood test), X-rays or electrocardiogram

Major Medical Plan (Comprehensive) AND Pet Wellness Plan Plus (Everyday care)

  • Once I enroll, how long before my policy is effective?

    Vet Helpline is a free service for all our members. Live veterinarians are available 24/7 to answer general pet questions and help identify urgent care needs. A Vet Helpline consultation is not a substitute for a visit to your primary vet.
  • What is a pre-existing condition?

    A pre-existing condition is any illness or injury your pet had before coverage started. While no pet insurer covers pre-existing conditions, the good news is that Nationwide doesn’t permanently exclude all such conditions. If you have medical records from your vet showing that your pet’s condition has been cured for at least six months, you may be able to get it covered.
  • Can I cancel my policy before the term is up?

    Yes, you can cancel any time by notifying us in writing. If we receive your written cancellation request within 10 days of the date your policy first became effective, we’ll refund your premium. (California residents: We’ll refund your premium if we receive your written cancellation request within 30 days of the date your policy first became effective.)
  • Will you drop a pet from coverage because of age?

    Never. As long as your pet is enrolled before age 10 and you keep your policy continually in force (translation: don’t let it lapse or expire), we promise not to drop your pet.
  • Are medical records required for enrollment?

    Not always. Sometimes we request medical records for clarification so we can ensure you receive all the benefits available under your plan.
  • What are my payment options?

    We offer monthly and annual payment options and accept credit cards, debit cards and checks. We also accept payment by payroll deduction; ask your employer’s benefits administrator to see if this option is available.
  • Does my pet insurance policy cover diagnostic testing?

    Yes. We reimburse for diagnostic tests as long as the medical condition is covered by your policy.
  • Are anterior cruciate ligament (ACL) injuries covered by my policy?

    We cover ACL injuries that are diagnosed after you’ve had your policy for 12 consecutive months. And unlike many other pet insurers, we cover both knees.
  • What if I don't know my pet’s exact age?

    No problem. Just take your pet to the veterinarian for a physical exam that includes an age estimate.
  • Are prescriptions covered under my policy?

    Yes. FDA-approved prescriptions for covered medical conditions are eligible for reimbursement.
  • How does the deductible work?

    A low annual deductible of your choice applies to our medical coverage plans (Major Medical Plan and Medical Plan). Once you meet your deductible, we’ll reimburse you up to 100% of your plan’s benefit schedule. There is no deductible for wellness coverage plans.
  • Is there a list of veterinarians I have to use?

    Nationwide, you’re free to visit any veterinarian, anywhere—even specialists and emergency providers.
  • How do I file a claim?

    It’s easy. Pay your veterinarian directly and then send us your claim form along with your itemized receipt. You can fax your claim to 714-989-5600 or mail it to Nationwide Claims Department, P.O. Box 2344, Brea, CA 92822-2344.
